We work as a team with each patient’s physician to create a personalized care plan. Besides the patient’s physician, the hospice team includes physicians, a medical direc-tor, nurses, certitied nursing staff, physical, speech and occupational therapists, social workers, spiritual advisors, and volunteers. A nurse is on call 24/7 to answer questions and ofter support.
